ENTRY FORM

            There are Two Sections:                       Closing Dates:    July 6, 2008 for online entry
            Section A: Open Category                                                 July 11, 2008 at the Conference by 5:00 pm
            Section B: Nature Category                                                    $10 Fee to enter on Jul 11.

Please read the Entry Rules before filling out this form.
Check the status of your submission.

INSTRUCTIONS:  To start, Click inside the field "Last Name".  Use the "Tab" key to advance to the next field.

FILENAME:  It is important to use the following format for your Image Filename:

            lastname-x-####-yz.jpg
                Where x is the first character of your given (first) name.
                Where #### is the last four digits of your phone number.
                Where y is the Section Letter (A or B).
                Where z is the image number for that section (this will always be 1).

            Example:  smith-j-1234-A1.jpg: Image # 1 entered in Section A by John Smith phone no. 727-555-1234
After you click on Submit Form below, you will need to send an email to necccdigitalcomp@verizon.net with your image files (as named above).
